開頭我想先說
這真的是一段既漫長 又繁雜的旅程
附上來回的信件內容供參考
以下開始用故事的方式敘述整段過程 以及該注意的地方
Step 1. 跟你的公司要負責人的連絡方式
在我離開我的工作之前
我先向Manager要了負責我薪資支出的人的Email
因為之後要向他詢問有關退稅以及退休金的事情
務必要在離開工作前要到聯絡方式
不然離開工作之後 再去要連絡方式會很麻煩

We had another person who was in charge of the super who stuffed up the payments. We are now working with the ATO to correct this and have just completed all the reconciliations but the amounts probably wouldn't have been allocated to your accounts.
I have made the payments for the March 2012 qtr but I still sending all the information to Hostplus so that the amounts can be allocated. This will be done by the end of next week.

I have only just sent through the allocations for March to Host Plus so the amount will not have gone to your account yet. The payment was however made a while ago.
I'm sorry I haven't been able to do the allocations but Bruce has been away so I have been doing everything.
The payment made to you for March was $XXX and I have sent the allocations now so it should be in an account for you within the next week. I will send through your account details once I have received confirmation from Host Plus.

Step 2. 跟負責人要退休金帳號密碼
回到台灣約1個月後 開始準備申請退休金
寄email去問公司負責人詢問退休金公司以及帳號

Thank you for your email.
Based on the information you have provided in your email, your account number is xxxxxxxxx.
However, there is a discrepancy with the date of birth details you have provided in your email as it do not match our record.
You may provide a signed confirmation letter from your employer on company letterhead confirming that a wrong date of birth has been provided to HOSTPLUS and confirming the correct date of birth. This letter should be signed by the registered contact of the employer account with your full name, date of birth and address details.
Alternatively, if you wish to update your account to reflect a change of date of birth, we require a certified copy of your birth certificate or driver’s license or passport. The certified document must display the original stamp and signature (including credentials), printed name, registration number, business name and location of the person certifying the document.
The photocopied document must be certified by an authorised person. To assist you with this, we have attached our guidelines which outline how to provide acceptable certified documentation. In order to maintain the security of your account we are unable to accept photocopies or faxed copies of certified documents.
We also need you to complete the attached Change of Member Details form and provide the following member information for security reasons:
§ Full Name
§ Date of birth
§ Previous address
§ Current address
§ Nominated beneficiaries (if applicable)
§ Employer who has made contributions to member’s account
Please mail the valid certified document and signed form to HOSTPLUS

這下好了 竟然說我的生日與他們資料庫裡的不相符 只給我帳號 不給我密碼
他們提供了2個解決方法
1.請原公司寄信給Hostplus說他們登記錯資料 並提供正確資料
2.自己提供駕照或護照 的certified copy 以及信中所提到的全名 生日 住址等資料並寄給他們
我先寄信給原公司負責人 希望他能寄信給Hostplus更改資料

果然 離開了工作之後 公司負責人根本不會鳥你 過了那麼久完全沒有任何動作
雖然我不死心 再寄了幾封信給公司負責人 但是都有如石沉大海一般 無聲無息
於是我決定自力救濟了
Step 3. 寄認證文件
寄送認證文件到澳洲給Hostplus更改資料
之前提到 認證文件可以是駕照或是護照 但為了避免麻煩 我選擇護照
首先先把護照資料頁拿去影印後 再拿給認證人員簽名認證
Hostplus允許牙醫 護士 法官 警官 任職於學校的老師....等
當然 你也可以去澳洲駐台辦事處 或是民間的認證機構(但是要花$$$)
要請認證人把以下的訊息手寫在要認證文件的正面(背面不行 會視為無效)
‘This is a true and correct copy of the original’
their name(印刷體), address, occupation(職稱), business hours telephone number and registration number(if applicable 這個不寫也沒關係)
and sign each page(簽名)
這樣就完成了一份文件的認證
接著就是把這份文件寄到澳洲去 我用郵局寄國寄掛號 NT 58
Hostplus當初給我的地址是

並寄了email給Hostplus

約莫過了10天之後 我收到回信了

原來早就弄好了 那幹嘛不早點跟我說 讓我花了一堆時間....
STEP 4.確認退休金帳戶裡是否有錢
嘗試了登入Hostplus 但是仍然失敗
所以打國際電話到Hostplus詢問
服務人員表示需要ID 姓名 住址 以及生日 認證完畢後
給了我一組臨時密碼
終於可以登入我的帳戶了!!
沒想到一登入發現 原本我應該收到的退休金(年初到6月) 但是我只收到年初到3月的
只好繼續去跟公司討錢了

但是走到這一步 也花了許多時間 帳戶裡面已經被扣掉不少錢了

在相應位置打X就好了
終於把表格填寫完畢 並附上護照上出入境頁面的影本並經過認證(跟上面步驟一樣)
再次把信寄到澳洲去

- You have not provided a certified copy of your passport showing the arrival date for Australia.
- We require a certified copy of your Birth Certificate to proceed. This is due to differing details between your ID and your account

此時我以為是我認證寫在背面的關係(我忘記認證寫在哪一頁了) 所以不接受認證
所以再次寄了護照上的資訊頁面給他

I wish to advise you that due to fund policy and for the security of your account we require a certified copy of your birth certificate translated in English. This is because your birth details on our system do not matching your passport. As the birth certificate is a legal document of you birth it is the only document we can accept to update your details. Once we have the correct details we can process your DASP application.
